HONESDALE, PA — Wayne Memorial Hospital (WMH) has again received the American Heart Association Gold Plus Get with the Guidelines Stroke Quality Achievement Award and the Rural Stroke Silver Destination Award for the second year. “The award recognizes the hospital’s commitment as a Primary Stroke Center (PSC) ensuring stroke patients receive stabilizing evidenced based stroke care and essential protocols for acute stroke management,” the hospital said.
WMH earned the award by meeting measures for the diagnosis and treatment of stroke patients at a defined level for a designated period. These measures include evaluation of the latest evidence-based and research-based recommended guidelines for use of medications and other stroke treatments with the goal of improving patient care and outcomes in the community served.
Before discharge, patients also receive education on managing their health, get a follow-up visit scheduled, as well as other care transition interventions.
WMH also received the Target: Type 2 Diabetes Honor Role Award. To qualify for this recognition, hospitals must meet certain quality measures. Target: Type 2 addresses heart disease and stroke risk factors in patients with Type 2 diabetes.

Wayne Memorial Hospital is a Certified Primary Stroke Center. This recognition is part of the American Heart Association’s ongoing work to improve heart and brain health and ensure quality care is available to patients no matter where they live, the hospital said.
