Two Liberty men arrested for forgery, theft

On Thursday May 24th, 2012 Village of Liberty Police in conjunction with the New York State Police arrested David Clark 37 and Jeanette Alvarez 23 both of Liberty after an investigation into the theft and forgery of a prescription from a Village medical center.
Police began investigating when the prescription was passed at Workforce Development in Monticello by Clark. The prescription was fraudulently filled out to allow Clark a week off from work. Police investigation revealed that Alvarez had stolen the prescription from the Medical Center in Liberty, fraudulently filled it out, forged the physician’s signature and gave it to Clark.
Alvarez was charged with forgery in the 2nd degree a felony and petit larceny a misdemeanor and Clark was charged with criminal possession of a forged instrument in the 2nd degree, a felony and criminal possession of stolen property in the 5th degree, a misdemeanor by Village of Liberty Police and was additionally charged with offering a false instrument for filing in the 1st degree by the New York State Police.
Clark and Alvarez were both arraigned in the Town of Liberty Court. Clark was remanded to Sullivan County Jail in lieu of $1,000 bail and Alvarez was remanded to Sullivan County Jail in lieu of $2,500 bail.
